Your Visit to the Office Please inform the receptionist of your arrival before being seated. Before you are seen by the doctor, you will be asked to complete a case history regarding your condition or problem. Some of this information will involve your past medical history. They supply the doctor with essential information about you. It is also important that we know the names of and dosages of any medications you have been, or are taking.

Appointments Patients are seen by appointment only. Please call in advance to reserve a time. If a problem arises with the scheduling of your appointment, we do appreciate two hours advance notice of cancellation. Note: We are currently not accepting appointments by email. Services Our health care is restricted to the specialty of Chiropractic. Within this specialty we deal primarily with the cause, treatment and prevention of diseases related to the dysfunction of the nervous system. Treatments include chiropractic, spinal decompression and nutritional consulting.
POLICY ON INSURANCE We will fill out and submit all forms for your convenience. Our office will verify insurance coverage by phone in advance to determine your benefits. Please remember, the insurance company is not responsible for the bill. This is the patient's responsibility. Any amount not paid by the insurance company is the patient's responsibility. Insurance and Fees Patients are responsible for all fees for services rendered. Your first visit must be paid in full, unless other arrangements have been made, or your insurance coverage dictates only a co-pay be made. After your initial visit, several payment plans are available. Medicare patients are responsible for examination and x-ray charges only unless you are covered by a secondary policy. We are medicare participating providers; so after your first visit, you will be responsible for 20% of the treatment charge unless you are covered by a secondary policy. Those patients without insurance are responsible for payment in full at the time the services are rendered. We accept Visa, Mastercard, American Express and Discover for financing of your treatment fees.
